Incontinence Slings
The “Slings” you hear about are for Stress Incontinence (leakage of urine that occur with laughing, coughing, sneezing, jumping, trampolines. Slings are made of polypropylene, an inert nylon, that is placed right under the mid urethra to act as a backboard when pressure from above (cough/sneeze/laugh/jump) compresses the urethral tube into the sling to prevent leakage. Slings are “Tension-Free” because the slings are free floating and not sutured into any structure. This is the current Gold Standard of incontinence surgery.
You may hear the term “TVT” or “TOT” which refers to the route the slings are placed. TVT, or tension-free vaginal tape is placed through incisions right above your pubic bone. TOT (transobturator tape) is placed through incisions on the crease of your inner thighs. Once healed you can barely see these incisions. Both procedures are outpatient surgeries of about 15 to 30minutes and can be done with pelvic floor repair and cosmetic procedures such as labiaplasty and vaginoplasty. The success rates vary from 80 to 95 percent. Occasional complications of mesh eroding through skin, urethra, or bladder do occur. Other complications are slings that are too tight preventing urination. I use a sling system I helped design back in 2002 which has proven to be safe, works well, and is enduring. They are shown in the example surgery below with my hands holding them.
The Luksenburg System for Mesh-Free Incontinence Treatment
Ariel Luksenburg, M.D., a gynecologist from Uruguay, introduced his system for the treatment of severe stress incontinence to the U.S. in 2021 at the International Society for Cosmetogynecology Pre-Congress workshop in Fort Lauderdale, FL. The course was repeated in 2022 and most recently in March of 2023. This time with FDA clearance to use his threads and patented delivery system. After hearing the system patent was granted, the PCL threads becoming CE and FDA compliant, and seeing the 3-year data presented, I became a believer. I was honored when Ariel Luksenburg and Jorge Gaviria invited me to become the first surgeon in the U.S. to be trained, perform, and to teach The Luksenburg System. They committed to also train my California colleague Judy Wei, MD. Cystocele Repair or Fallen Bladder Repair
A bulge of skin that can be felt or seen coming out of the vagina looking like an egg or a golf ball is usually the bladder or rectum. Sometimes it is the urethra, cervix, uterus, or top of the vagina if there has been a hysterectomy. Urinary leakage often accompanies this bulge if it is the bladder falling down along with the tube that drains it called the urethra. Us gynecologists call this “Prolapse.” Pelvic pressure, feeling of fullness and heaviness are common symptoms. This is mostly caused by childbirth trauma and the aging process with loss of collagen support. The average age of my repair patients is 54. Sometimes there are minimal to no symptoms and you just see an unsightly mass. Cystocele repair (also called Anterior Repair or Anterior Colporrhaphy) is the surgical elimination of the bulge and restoring normal anatomy. This is done in the Surgery Center or hospital under general or spinal/epidural anesthesia. Traditional techniques of bunching or overlapping tissues with suture has a very high failure rate ranging from 25 to 60 percent. It is a challenging surgery. Modern repair treats this bulge as a hernia of the bladder going into the vagina aided by a graft to build a new firm wall. I have performed and taught this augmented surgery in the United States since 1997 with success rates of about 90% when using a dermal patch (cadaver skin). I currently use the product made by ARMS Medical called VNew Dermal Allograft. I designed the specialized shapes for its bladder repair and rectal repair products. The use of nylon patches works very well with failure rates less than 5% but the FDA has removed most of the nylon mesh products in the market. A company called Coloplast has an ultra-lightweight mesh that experienced Urogynecologist have used safely and successfully. I brought this into the United States from Scotland in 2004. It was first called POP Mesh, then MPathy Mesh, and now Restorelle. I may use this product for the most severe cases or use it for prior failed surgeries.
Rectocele Repair or Fallen Rectum Repair
When the bulge into the vagina comes from the rectum it is called a rectocele. As with other forms of pelvic organ prolapse (cystoceles, enteroceles, vaginal prolapse) childbirth, chronic cough, chronic constipation, and obesity are predisposing factors. Symptoms are similar to cystoceles such as pelvic pressure, an unsightly bulge in the vagina, and constipation. It is sometimes hard to tell if the bulge is from the bladder or rectum and the examining physician will need to use a “Half Speculum” to define what is hanging out. Furthermore, the need of reaching into the vagina to push stool out is not uncommon and a giveaway that the rectum is falling out and giving symptoms. Of course, both the rectum and bladder can fall out together at the same time. Surgical repair consists of using sutures to bunch up the bulging tissues together or bringing muscles together into a new flat floorboard. More modern repair consists of the use of mesh or donor tissues. Rectocele repairs have a 90-95% success rate when the muscles are approximated together.
Uterine Suspension
When a hard lump is felt or seen hanging out the vagina it’s usually a cervix and uterus and often with a bladder attached. A hysterectomy and suspension of the top of the vagina is the traditional cure. It works very well. If you don’t want a hysterectomy, then I can suspend the entire structure falling out with a suspension procedure I invented almost 20 years ago. This is a great option for those with a small cervix and a small uterus typical of menopausal women. I have taught this technique worldwide. I use either cadaver tissue or ultralightweight mesh as suspensory and supporting pseudo-ligaments like a pulley system. I do these in a surgery center or operating room under general anesthesia. It takes me 60-90 minutes. Again, a high-volume surgeon for this specific procedure is important to find if you want the best outcomes.
Vaginal Vault Prolapse (Fallen Vagina)
After a hysterectomy a vagina that loses its support may come down and out like an inside-out sock. If a woman still has her uterus then this is called a uterovaginal vault prolapse. If only the uterus falls out and the top of the vagina is still well suspended then it is called a uterine prolapse. Vaginal vault suspension can be done in many ways. You can use the patient’s own weakened tissues or augment the repair using a biologic product such as human cadaver skin dermis or polypropylene nylon. Some physicians prefer an abdominal approach to attach the top of the fallen vagina to the sacrum. Some highly skilled surgeons do this laparoscopically with a Sacralcolpopexy. That is when you suture the top of the vagina to a bony prominence of the sacrum deep inside the pelvis.
Robotic surgery has made this easier this past decade and today’s recently trained graduates use this route as their primary approach. The vaginal approach to suspending the top of the vagina used to be the most common but it is becoming less and less as their surgical numbers in training programs decline. The top of the vagina can be sutured to the uterosacral ligaments or to the sacrospinous ligaments through the vaginal approach. Either approach works well with different complications to consider. The vaginal surgery skills needed for this excellent technique is getting lost in today’s academic training programs. You have to find a high-volume surgeon that gets enough of these cases to remain competent. The success rates of all methods are approximately the same at 80 – 90%.Social Media:

BACKGROUND on LABIAPLASTY:
In 2005 I developed the Barbie Look Labiaplasty, a radical and aggressive form of labiaplasty that removed almost all to all of the labia minora to achieve maximal comfort for athletes, reduce the outlining of the labia when wearing swimsuits and leotard, and as a cosmetic preference to obtain a “clamshell” look wanted by women in Southern California. This was an athletic, smooth, and sleek look that was the most “tucked in” you could get. It was impossible to achieve this look with the standard plastic surgery wedge labiaplasties. It has been a beloved surgery for the carefully selected patients who want this minimalist “Look” it. No more adjusting and tugging. My Olympic level athletes found it to be an outstanding choice.
HISTORY:
Soon after my Barbie Look Labiaplasty technique became well known through my teaching programs, the media, presentations in Congresses, it was copied worldwide and attempted by untrained surgeons. Poor and awful outcomes were rapidly occurring, and I was referred many patients for repair. Patients came to me with no labia minora at all when they did not seek out or ask for the Barbie Look Labiaplasty. Their surgeon chose the wrong procedure of a Barbie Look for patients who wanted labia left. Many surgeons who had done zero to only a few labiaplasties ended up not knowing of the massive retraction of tissues in the upper third of the labia minora with resultant retraction of labial edges to end up flat and with NO labia at all. These surgeons did not know what to do and I was often consulted. I was also sought out as an expert witness for malpractice cases.
I had a small handful of patients in the first decade of my practice doing the Barbie Look Labiaplasty who changed their minds months to years later and wanted to have a little bit of labia minora showing and not be completely flat. The pictures below is one of those patients. My original solution was simple and worked well if they wanted just a little bit of labia minora below the level of the labia majora. If they wanted more of the labia above the level of the labia majora I would have to make labia majora incisions up to 2cm above where the labial crease would have been. I called this my Grooving Technique because I created a groove between where the labia minora and labia majora had met, the interlabial fold, undermined medialy, and created a labia from no labia. It was actually creating an optical illusion of a new labia when in reality it was creating a groove and sometimes suturing the lateral majora edge to the lowest portion of minora edge that was just “grooved.”
I shared this secret with Amr Seifeldin when he visited my office on October 6, 2015, 2017, 2019. I did not publish my technique and only shared it with my Alinsod Fellows who would travel to Laguna Beach over the past 18 years to participate in my Preceptorships. Amr expanded on my methods for the patient who wanted a more significant labia minora reconstruction without the use of flaps and has taught this to his Fellows. His Center in Cairo, Egypt, has been a tremendous resource for patients in the area wanting both Female Genital Mutilation Repair, Cosmetic Gynecology Surgeries, and advanced Revision Labial Surgeries such as my Grooving Technique. The landmark article by Reham Awwad, Amr Seifeldin, Natalie Sorial, and myself is attached at the end of this newsletter.

PATIENT SELECTION AND GROOVING TECHNIQUE SUMMARY:
1. For the patient who wants to create the illusion of a labia minora where there was none up to the level of the labia majora and just past it.
2. Local anesthesia
3. Use radiosurgery to create a “Groove” between the labia minora and labia majora, where the crease used to be, the former intralabial fold.
4. Start with the fine pinpoint “Hair Tip” of the Ellman or Soniquence RF to outline and incise from the top of the labia minora to the introitis.
5. Use this “Hair Tip” to smoothen any labial and hood areas to create a smooth from from the clitoral hood to the frenulum or its remnants, to the labia minora.
6. Use the small ball tip to continue the Grooving deeper while maintaining hemostasis to create a flab of labia minora medially. With adequate numbing cream, a QTip is used twice daily to keep the edges from fusing together. This is performed for 4-6 weeks. No suturing needed.
7. For more significant labial creation, suturing is need to attach the lateral majora edge of the incision to the lowest portion of the medial minora edge to keep the appearance of a new labia minora.
8. Inject PRP into the surgical site and use Estrogen Cream or Exosome Cream to aid in healing for the next 6 to 8 weeks.

Understanding the Gaping Vagina and Queefing
What is a Gaping Vagina? A "gaping vagina" refers to a condition where the vaginal opening or canal becomes unusually wide or loose. This can lead to a decrease in sexual satisfaction, physical discomfort, and emotional distress due to changes in body image. Queefing is vaginal flatulence or gas. These two often travel together.
Causes from Childbirth Trauma:

A Journey Through Time: The History and Global Rise of Cosmetic Gynecology
Cosmetic gynecology might seem like a modern trend, but its roots stretch back further than you might think. For centuries, cultures around the world have explored ways to enhance or restore the female body, often for aesthetic, cultural, or functional reasons. From ancient practices in Egypt and India—where genital modifications were tied to rituals or beauty standards—to the more medical approaches of the 20th century, the desire to address women’s intimate health and appearance has always existed. But it wasn’t until recent decades that cosmetic gynecology emerged as the dynamic, specialized field we know today.
The modern story begins in the late 20th century, when pioneering doctors started blending gynecology with cosmetic surgery techniques. In the United States, visionaries like Dr. David Matlock introduced terms like “vaginal rejuvenation” in the 1990s, sparking curiosity and debate. Around the same time, Dr. Marco Pelosi III and his father, Dr. Marco Pelosi II, coined “cosmetogynecology,” laying the groundwork for a new frontier in women’s healthcare. Dr. Red Alinsod coined the procedures and phrases such as “Hybrid” and “Barbie Look” Labiaplasty and ThermiVa. What started as a niche idea—focused on procedures like labiaplasty or vaginal tightening—quickly gained traction as women sought solutions for both aesthetics and comfort after childbirth, aging, or simply personal preference.
Fast forward to today, and cosmetic gynecology has exploded into a global phenomenon. From North America to Europe, Asia, and beyond, the field has grown thanks to advancements in technology—like lasers and radiofrequency treatments—and a growing openness about women’s health. In India, for example, demand for these procedures jumped from just 3.9% in 2012 to nearly 29% by 2015, reflecting a cultural shift toward empowerment and self-care. In Europe, non-surgical options like fillers for vulvar rejuvenation are all the rage, while Brazil and Argentina have become hubs for cutting-edge techniques. This worldwide boom isn’t just about beauty—it’s about confidence, function, and giving women choices.
The International Society of Cosmetogynecology (ISCG), founded in 2004 by Dr. Marco Pelosi III and others, has been a driving force in this evolution. It’s where experts from every corner of the globe—like those I just met at our annual meeting in Fort Lauderdale—come together to share breakthroughs, refine techniques, and ensure these procedures are safe and effective. What began as a small group of innovators has grown into a vibrant community, mirroring the field’s own expansion. Today, cosmetic gynecology isn’t just a medical specialty—it’s a movement, empowering women to feel their best, inside and out.
As I reflect on our recent gathering, with Dr. Pelosi III’s inspiring “State of the Art” message and the memories captured in my photos, I’m reminded of how far we’ve come—and how much more there is to explore. My Bread-and-Butter Surgery is Labia Minoraplasty. Labia Minoraplasty reshapes the thinner inner lips that can cause irritation, rubbing, pulling, and painful sex. Many professional athletes and active women have this surgery to reduce the need for reaching down and chronically adjusting like baseball players do. This is also often done to improve the appearance and symmetry of the labia minora and help improve confidence. In reality most patients want it to look better and feel better too. The term “Labiaplasty” is also cosmetic surgery of the labia majora (larger outer lips) to make it less prominent and saggy.
Almost all our labiaplasty surgeries are done in the office awake and without an IV using local anesthesia. You can choose to take gentle antianxiety pills and a mild narcotic to be comfortably warm and fuzzy during the procedure. Precision radiofrequency surgery or LightScalpel laser surgery are my preferred tools. I created a lay person lingo to describe the degree of labial possible.
I do curvilinear excision and avoid wedge labiaplasty in general. Significantly fewer complications and breakdowns. You won’t find my terminology in most medical textbooks. For example, removal of just the edge of the labia is called Rim Look Labiaplasty. It is the most conservative aproach. Removal of almost all or all of the labia minora is called The Barbie Look (Barbie has no labia). I developed this in 2005. This is the most aggressive approach popular in California and the West Coast and the most requested look in my practice. It is certainly not for everyone. Patients who have enough labia majora to be able to cover the opening, those who want ultimate comfort without the need to chronically adjust themselves (athletes and Olympians) are great candidates for this Look. You have to be sure and not wavering or second guessing before you choose this technique because you cannot reverse it.
The middle road where most of the labia is removed but leaving some is called The Hybrid Look. It competes with the Barbie Look and almost indistinguishable from it as its edges are also below the level of the labia majora also and looks tucked in but not all gone like the Barbie Look.
Of course, the patient can choose what degree of tissue removal is done as I have the most precise tools to achieve desired outcomes. Labia minoraplasty takes me a couple of dedicated and intense hours of concentration. Run away from the surgeon that tells you they can do this in half an hour. You want precision.

As more gynecologists, urologists, plastic surgeons, and cosmetic surgeons perform labial and vaginal surgeries, the need for revision surgery has steadily risen. Most of these surgeons attempting labial and vaginal surgeries have had no formalized training in aesthetic vaginal surgeries. This leads to complications, displeasing appearance, and unhappy patients. An example is a very notable porn star who allegedly had a botched vaginoplasty performed in Beverly Hills by a renowned surgeon. More and more anecdotal reports have surfaced about less than optimal results following labiaplasty surgery such as asymmetry, excess removal of tissue, disrupted blood supply, and pain. Vaginoplasty patients report painful intercourse, tightening only of the entry, lack of deep tightening, and recurrence of looseness. This has given established labial and vaginal surgery experts more secondary attempts at repairing improperly done or aesthetically displeasing surgeries. This has also given a select group of surgeons more and more legal consultations from patients and lawyers alike as the standard of care evolves and grows. In our center, a full 10% of work is done as revision surgery. The number continues to rise as more revisions, resurfacing, re-tightenings are asked for. It behooves a surgeon thinking of perfoming labiaplasty and vaginoplasty surgery to be properly and completely trained before attempting these intricate surgeries.

Great things are happening in Dallas! Over 10 years ago I designed and helped launch ThermiVa Radiofrequency treatments and it became the standard for Feminine Wellness and non-surgical “Vaginal Rejuvenation.” Now we take a quantum leap forward. After 7 years of an intense approval process, on January 21, 2025, Compex of the Czech Republic, and their Jett Medical device, received their FDA 510k clearance in the United States. Let me explain why this is so important for the field of Cosmetic Gynecology.
* Plasma technology: DC current (Direct Current) in the 1-5 milliampere range is able to create non-ablative plasma energy at the Silver tips. This stimulates comfortable heat up to 42 Celsius, stimulates fibroblasts and tightens skin, results in increased blood flow and moisture, collagen formation, elastin formation, and nerve sensitivity improvement.
* Plasma technology: DC current creates plasma energy at the Gold tips for ablation and shrinkage of lesions and skin. Treats HVP/Warts/Molluscum/Angiomas. Silver tips are non-ablative and can tighten skin without any burns.

Jett Medical Plasmaporation treatments and results are very similar to radiofrequency ThermiVa and much more comfortable than micro needling Morpheus 8 or lasers such as Mona Lisa and Diva. Its huge advantage is the ability to open water channels and push biologics such as PRP and Exosomes under the skin and enhance tissue engineering. No other device can do both.

WHAT IS JETT MEDICAL: European/Worldwide Indications
A patented direct current electrocautery device indicated for non-ablative treatment of vulvovaginal laxity causing tissue tightening and strengthening. Unique direct current technology provides a solution for issues connected to vulvovaginal laxity, such as insufficient vaginal moisture, burning and itching, stress incontinence and sexual discomfort due to vaginal dryness, reduced tightness, elasticity and sensitivity. Developed mainly for women after childbirth and menopause.
MECHANISM
The positive effect of JETT MEDICAL II is caused by combination of heat, membrane depolarization and low threshold reversible electroporation. The passage of direct current generates heat, which activates the formation of new collagen connective tissue, leading to a rejuvenation and stronger and more flexible tissue structure. In addition to the heat effect, unique direct current technology causes membrane depolarization and reversible electroporation which reduces the barrier and enable easier passage of ions and molecules through cell membranes.
The procedure is painless, well tolerated and it does not require local anaesthetic application. It solves rejuvenation of atrophied vaginal mucosa. It is suitable also in cases when local estrogen vaginal creams cannot be applied (condition after oncological diseases) with estrogen-dependent tumours.
VULVOVAGINAL TREATMENT PRINCIPLE
Small micro-discharges occur in the gel between the applicator and the tissue. Micro-discharges cause local increase of electric intensity. When this intensity reaches certain threshold value, the electroporation appears. Micro-discharges randomly arise and disappear in huge numbers, which ensures desired effect in the treated area.
Local increase of electric intensity causes nanopore formation in the cell membrane. Macromolecules and ions can easily get into the cell through these pores.
- The cells are refreshed and increase its volume.
- The atrophy of vaginal mucosal and submucosal tissue decrease.
- The cohesion between mucosal and submucosal tissue is improved.
After treatments using Jet Medical in Europe and outside the US, patients reported significant improvement in their sexual quality of life with reduction of incontinence, urgency, and atrophy. First of all a Happy New Year to all my readers and subscribers! This is my first Newsletter for 2025 and it is very special. Lots of stuff in this one.
I'm Dr. Red Alinsod, and I want to talk to you about something that might be bothering you - external anal skin tags. Now, these are not something to be too worried about, but I know they can be a nuisance or even a little embarrassing. Few doctors take care of these problems. GYNs, General Surgeons, Colorectal Surgeons, Gastroenterologists mostly run away from these surgeries because they are typically not covered by insurance. I decided to make this a major focus of my practice about 20 years ago. In Dallas where I practice, I have pursued the woman who wants to get rid of anal skin tags. Patients from just about every state fly into Love Field or DFW Airport, drive a few miles, get these removed and they fly home in the afternoon or evening!
Today’s Newsletter will focus on clinical outcomes and what you can expect from surgery to remove those Anal Skin Tags, aka “AST,” using Radiofrequency surgery (RF). I have taught hundreds of doctors this technique around the world. I was the first to present this surgery in any Cosmetic Gynecology/Gynecology/Plastic Surgery Congress. More anal skin tag surgeons have this less expensive radiofrequency technology (made by Ellman or Soniquence in the USA) and this is what I used to teach my Fellows in the past 18 years. In my personal practice I have transitioned mostly to the CO2 Laser made by LightScalpel. It is a very expensive laser so few practices own one. I developed the protocols for LightScalpel for its use for gynecologic and AST surgeries. Women seek us out because of this laser and the comfort and rapid healing and beauty it can provide. Both Laser and RF technologies work well and now, between the two newsletters, you get both sides of the story.
We are the best and busiest Anal Skin Tag center in Texas and want to brag a bit of what we have to offer our patients. This is an update on my newsletter published on February 1, 2024: Those Pesky Anal Skin Tags. I focused on using the LightScalpel Laser on that video shown below:
Let me do a quick summary first:
What Are ASTs?
External anal skin tags are just little bits of extra skin that hang around the outside of your anus. They're benign, meaning they're not harmful or cancerous. They look like small flaps or bumps, and they're usually the same color as your skin or a bit darker. They're pretty common, especially if you've had hemorrhoids or anal fissures in the past. When those heal, sometimes they leave behind these little skin tags.
How Do They Occur
These tags can pop up for a few reasons. Often, it's because of the stretching and irritation from conditions like hemorrhoids or fissures. After the swelling goes down, the skin doesn't always shrink back to how it was, leaving these tags behind. Pregnancy, obesity, or even just the natural aging process where the skin loses elasticity can contribute to them. Low fiber diets and lots of red meat eating can make things worse. Sometimes, it's just genetics playing a part.
Treatment Options
Now, if these tags are bothering you - either because they're itchy, uncomfortable, or you're just not happy with how they look - there are ways to deal with them:
- **Watchful Waiting:** If they're not causing trouble, you might decide to leave them be. They're harmless unless they get irritated. If the appearance does not bother you just leave them alone.
- **Home Remedies:** Some people use over-the-counter remedies to minimize symptoms like itching (Preparation H, Anusol, Hydrocortisone). They can give relief and shrink them down a little bit. I always caution against trying to remove them at home yourself. It's not safe and can lead to bleeding, infections, or worse.
- **Professional Removal:** This is where I come in. At the Alinsod Institute, we offer specialized treatments for skin tag removal. We do not use rubber bands. We use techniques like:
* Laser Surgery: The least invasive option where we use a CO2 laser to excise and vaporize the tags. This technique is hard to find because of the expensive laser. Make sure the practice has a LightScalpel laser because not all lasers are as effective. This method is the best for its precision and minimal discomfort when done by an experienced surgeon with the right tools. Rare scar formation.
* Radiofrequency/Electrocautery: Using radiofrequency energy to shrink and remove the tags. This method helps control bleeding and speeds up healing. Usually no suturing is needed so much less chance of scars. An excellent method I use. I use the Soniquence system. They have the best Bipolar system
* Excision with knife: Cutting them off with a clamp and a knife under local anesthesia. It's quick, and you can go home the same day. More scars because of frequent need of suturing. More pain due to the sutures used. I do not use this method.
Summary of Risks/Benefits/Options
* Risks: pain, bleeding, infection, scars, recurrence, need for repair or redo, they do nothing for the internal hemorrhoids. You can have reactions to medications given for pain and comfort such as an allergic response.
* Benefits: comfort, stop the bleeding, cleanliness, beauty, confidence.
* Options: leave them alone, topical creams to shrink them or reduce inflamation, rubber banding, consult with another surgeon, surgery with laser or radio waves or knife.
Preparation
There's not much you need to do before coming in. Evacuate your bowels, keep the area clean, but you don't need to do any hair removal or special prep. You do not want to do this surgery if your hemorrhoids are acting up, bleeding, and engorged. Anal skin tag Surgery is for when the hemorrhoids have calmed down, not inflamed or bleeding, and are only external and outside the anus.
Procedure Details
We'll do this in our procedure suite. Not the hospital or Surgery Center in most cases. It's a quick, in-office procedure under local anesthesia. I do them in the Surgery Center upon request such as severe anxiety or a serious medical condition. We have a magical device called “electroporation” that opens up your skin’s water channels and pushes concentrated “BT” (Bupivacaine/Tetracaine) numbing cream deep to make it numb so you do not feel the needles. Go to this April 1, 2024 newsletter calls “Needle Free Non-Injections” for all the details of the technology. It is very cool! I developed the protocols for gynecologic use over a decade ago. After electroporation you then get the local anesthesia injection that has bicarbonate in it so there is less burning sensation. I will use the LightScalpel or Soniquence RF for precision surgery, which minimizes pain and speeds up healing. If needed I will use Monsol Solution to take care of any minor bleeding areas. It replaces suturing. It takes about 30 to 60 minutes of total time, depending on how many tags we're dealing with. The actual surgery is probably only 10-15 minutes. The two videos I provide in this newsletters gives you lot of details about my technique.
Pain Management
The best way to manage the post op discomfort is to use the 30% numbing cream we will provide. It is wonderful “BT Cream” mentioned above. You can use it on a regular basis when you feel uncomfortable. You will not get toxic doses. That and the use of ice packs will provide pain control and reduce the puffiness. Pain management is crucial, and you'll get prescriptions for that. Typically it is Ibuprofen and a mild narcotic such as Dilaudid or Tylenol #3 for the times when the cream alone is not enough. Lastly, if you want to get about 3 days of numbness (let’s say you are traveling to Dallas then home by plane or long car ride) then consider the use of Exparel. It is a local injection I can give you at the end of the case that provides long term numbness (3-5 Days on average). It is made of Bupivacaine local anesthesia surrounded by a liposome/fat coat for slow steady release.
Recovery
Post-surgery, you'll likely be advised to take it easy. You can drive immediately after surgery and do all the normal life stuff if you did not receive any oral or injected medications (e.g. Ativan, Valium, Demerol, Dilaudid, Zofran). You must be on a high fiber diet with fiber supplements for a month to avoid straining during bowel movements, which can be uncomfortable with fresh surgical sites. Veges, fruits, nuts, legumes, roots, prunes, prune juice. I insist on Milk of Magnesia daily to keep the stools the consistency of toothpaste. OTC Fibercon, Metamucil, Colace are also quite helpful. You do anything and everything to keep that stool soft but not watery. Do not just go on a liquid diet. It is not healthy in the long run.
Post-Care
You can go to the gym and exercise to your heart’s content if you can tolerate the discomfort during healing. I recommend using the numbing cream before exercise and not to use the bike or anything that pushes on your vaginal and anal areas. You can resume sex at 6-8 weeks once fully healed. You may spot and have light bleeding so wear a panty liner.
We don't just leave you after the procedure; we're there for follow-ups, to answer questions, or deal with any concerns. We want you to feel confident and back to normal as swiftly as possible. We will call you regularly and make sure you are doing well. You will have my personal Private Patient number. You will have Dian’s number. We answer at all hours of the day or night. We answer texts and emails immediately. We can Zoom or FaceTime too.

How do I get in?
Call the office, set up a phone/FaceTime/Zoom or live appointment. Send in your photos securely if it is a virtual appointment. Fill out the forms my staff send you and I will review them and see if you are good to go for an in-office procedure. If you have high anxiety or medical issues needing a higher level of care we will do your procedure in the Surgery Center. You have to pay for the surgery center costs. Many women have the problem of a wide and gaping vagina after having had childbirth, even if they've only had one baby. Chronic pushing and constipation, and perhaps even heavyweight bodybuilding and lifting, can push the pelvic organs forcefully out of the vaginal canal and result in a vaginal opening that looks wide open. Sometimes there's even a little bulge from the rectum or the bladder. What can you do? If you have a fallen bladder or rectum (cystocele or rectocele or both), those can be fixed with surgery. If you don't have a bulge, and it is just gaping and looking wide open from the tear of a vaginal birth or episiotomy, then that can be repaired and reduced using a procedure called a perineoplasty where the space between the vaginal opening and the rectal opening is repaired and even tightened if needed. A full length, vaginoplasty or vaginal tightening surgery can also be done for more severe cases. This example shows a minimally invasive surgery done in the office that is able to reduce the appearance of a gaping vaginal opening by removing a small amount of vaginal wall tissue right near the opening of the vagina. Camel Toes refer to the outline of the labia majora (the outer lips of the vulva) that becomes visible through tightly fitting clothes, resembling the shape of a camel's toe. This phenomenon can occur due to anatomical factors combined with the tension of fabric in the crotch area which can be exacerbated by aging and weight loss. This video shows you how I remove the fat.

You are back from the vacation. The kids are back in school. And now it is time for self-care. For over 20 years Autumn has been my busy time for Cosmetic Gynecology. That leads me to begin this series on the various procedures I perform here in Dallas. I will make the videos bite sized and with enough information but spare you the gory details.
Combination surgery to reduce the Labia Majora (larger outer labial lips) and Labia Minora (smaller thinner inner lips) as well as the Clitoral Hood can be done at a single In-Office Awake No IV surgical morning. I first reduce the Larger Labia Majora and Clitoral Hood. Then I make the Labia Minora to be smaller and reduced to the size requested and I tuck it all in as symmetrically as possible. My short 2 minute video shows how.
