In this episode, Dr. Bohn discusses the subject of grief and how patients with terminal diagnoses may approach the holiday season differently once they receive their diagnosis.
Being informed that your time is short is devastating. Managing the stress of the holidays is challenging and the added worry knowing you are Terminal increases this significantly. Patients facing the Holiday Season after being told that this will more likely than not be the last one, evokes strong emotions. Sometimes, people find a way to find joy, peace, and hope as they express their love to others. Patients that manage to navigate the powerful emotions that are common facing this type of difficult situation report an unexpected freedom. There can be a reduction in typical holiday stressors as patients are given unexpected privileges and liberties by loved ones. There are terrible days and better ones, as anticipated during this stressful time of the year. I hope if this situation is facing you or your loved one, that you find some helpful information here to lighten your burden.

In this episode, Dr. Bohn discusses Ascites and Liver disease.

Ascites is the buildup of fluid inside of the abdomen. Typically this is due to Liver disease and the most common cause of the liver damage is due to Cirrhosis caused by alcohol. There are certainly other medical conditions that can cause Ascites. It could also be from a type of cancer, or inflammation (infectious causes such as hepatitis or Tuberculosis or noninfectious diseases such as an autoimmune related process affecting the liver). Other causes include: heart disease, pancreatic issues, Liver Vein blockages, and even some dialysis patients will develop ascites.

In this episode, Dr. Bohn discusses Connective Tissue Diseases which are inherited. When a person thinks about joint disorders they might not realize that some causes of joint deterioration and deformity can be caused by a disease state that also can affect internal organs. Connective Tissue is throughout our body and if the ligaments, tendons, bones and cartilage are affected by a disease process. Nearly every connective tissue disease has joint pain and swelling as primary symptoms. But there can be worrisome changes occurring on the molecular and cellular level behind the joint symptoms and there is risk for the disease to affect the persons internal organs.

This is a review information of Palliative Care (Symptom Management) with a few comparisons to Home Health and discussion of Hospice care. If you are not aware of Concurrent Care, you will want to pay special attention to the segment that discusses this valuable service. The is a short review regarding conversations pertaining to care goals and direction is important. The discussions need to be ongoing and both parties need to allow time needed for these sensitive topics.

In this episode, Dr. Bohn broaches the topic of CPR, and discusses some reasons why patients may choose to sign a DNR - or Do Not Resuscitate order instead of opting for LSTs (Life Sustaining Treatments). Being asked about receiving CPR is actually a normal and common question that is asked of patients admitted in the hospital. It is not intended to frighten patients but as disease states advance and patients realize that cure may not be possible, many prefer to be allowed to have a natural death. Choosing to not have resuscitation attempted is a common choice. Patients and families that have been educated about their advanced disease stage understand their clinical situation like to know they have options and can still be offered things they can accept.

In this episode, Dr. Bohn introduces the subject of Pain, with the intent to release several additional episodes which will be targeted to more specific avenues related to Pain as patients near the end of life. Pain from serious illness limits more than a person's activity. It affects mood, appetite, sleep and function of the person. It can cause dysfunction of the entire body as it can change the way several organ systems perform their daily duties. We are considering Clinical pain for this discussion today. This episode reviews Somatic (skin and muscle sensations) and Visceral pain (associated with internal organs). These types of pain could have Nociceptive or Neuropathic pain. Common descriptors of pain include: dull, sharp, aching, shooting, stabbing, throbbing, cramping, numbness, tingling or burning. It might be that the pain is described by a couple of these, but certainly not all. The patient's description of the character of the pain leads the clinician where and how to evaluate to find the cause of the pain.

In this episode, Dr. Bohn discusses a new medication on the market intended to treat people with a dementia diagnosis. The dread of the diagnosis of this disease is due to the limited treatments that are available for patients. Lecanemab was approved early in 2023 by the FDA for use and many insurance companies are developing protocols for the approval of this medication.

It is an outline for a living will that addresses Medical, Personal, Spiritual and Emotional needs, thoughts, and beliefs. 5 Wishes provides a conversation guide for patients and their loved ones in an attempt to leave no stone unturned. a tremendous asset and guide for people. These are such difficult topics and not something that we discuss everyday. Having this so well organized and setting aside specific time to review is a must for persons with serious illness. I suggest that this be used by each family member to document their thoughts, feelings and beliefs.
